    • Premature/full term neonates:

      • PO 1 unit/kg/dose OD
      • Max: 200 units/day in 1-3 years, 300 units/day for 4-8 years; 600 units/day for 9-13 years; 800 units/day for 14-18 years
  • Postherpetic neuralgia (Off-label):
    • PO 400 units BD/QID
  • Tardive dyskinesia (Off-label):
    • PO 1600 units OD
    • Best response if tardive dyskinesia <5 years
