• Hypoprothrombinemia:
    • Vit K deficiency: PO 2.5-25mg *1 OR SC/IM/IV 10mg*1
  • Anticoagulant-induced: PO 2.5-10mg *1 OR SC/IM/IV 1-10mg *1
    • May repeat PO dose in 12-48 hours or SC/IM/IV dose in 6-8 hours based on INR
  • Dietary supplementation:
    • Males: PO 120mcg/day
    • Females: PO 90mcg/day
  • Hemorrhagic disease of the newborn:
    • Prophylaxis: IM 0.5-1mg within 1 hour of birth
      • May repeat dose in TDS/QID
    • Treatment: SC/IM/IV 1-2mg/dose/day
      • May require higher doses if mother has been receiving oral anticoagulants
  • Warfarin stabilization (Off-label):
    • PO 100mcg OD/BD
    • Give with warfarin for patients on long-term treatment with variable INR
  • Injection:
    • 2mg/mL
    • 10mg/mL
  • IV route should be used only if administration by another route is not feasible

  • Taste changes (SC, IM or IV use)

Severe reactions including death have occurred during and immediately after IV injection even with precautions to dilute injection and avoid rapid infusion
