• Prophylaxis of macrocytic anemias associated with vitamin B12 deficiency:
    • IM 1g every 2-3 months
  • Pernicious anemia and other macrocytic anaemias without neurological involvement:
    • IM initially 1mg 3 times a week for 2 weeks, then 1mg every 2-3 months
  • Pernicious anemia and other macrocytic anaemias with neurological involvement
    • IM Initially 1mg OD on alternate days until no further improvement, then 1mg every 2 months
  • Tobacco amblyopia:
    • IM initially 1mg OD *2/52, then 1mg twice weekly until no further improvement, then 1mg every 1-3 months
  • Leber’s optic atrophy:
    • IM initially 1mg OD *2/52, then 1mg twice weekly until no further improvement, then 1mg every 1-3 months

Methylcobalamin is a cofactor for the enzyme methionine synthase, an essential enzyme in the formation of methionine from homocysteine which is essential the synthesis of purines and pyrimidines needed for DNA synthesis and red blood cell formation
