      • Induction: IV 0.6mg/kg/dose *1

    • 3 months-14 years:
      • Induction: Initially 0.6mg/kg IV
      • Maintenance: IV 0.075-0.125mg/kg IV OR continuous IV infusion 0.012mcg/kg/min
    • >14 years:
      • Induction: IV 0.45-0.6 mg/kg IV
      • Maintenance: IV 0.1-0.2mg/kg repeat PRN OR continuous IV infusion 0.01-0.012mg/kg/min IV
  • Rapid sequence intubation:
    • IV 0.6-1.2 mg/kg/dose *1
  • Neuromuscular blockade:
    • Induction:
      • Adults: IV 0.45-1.2mg/kg/dose *1
      • <17 years: IV 0.6mg/kg/dose *1
    • Maintenance:
      • Adults: IV 0.1-0.2mg/kg/dose PRN OR 10-12mcg/kg/min, then 4-16mcg/kg/min
      • <17 years, isoflurane/nitrous oxide anesthesia: IV 0.15mg/kg/dose PRN OR IV 7-10mcg/kg/min
      • 3 months-14 years, halothane anesthesia: IV 0.075-0.125mg/kg/dose PRN OR 12mcg/kg/min
