• Acute metabolic acidosis:
    • 2-5mEq/kg IV infusion over 4-8 hours depending on the severity
  • Chronic metabolic acidosis:
    • 90-180mEq/L (~7.5-15g) at a rate of 1-1.5L (first hour) 
    • Chronic renal failure:
      • PO 1625-2925mg/day divided every 4-6 hours
    • Distal renal tubular acidosis:
      • PO 42-168mg/kg/day divided every 4-6 hours
    • Proximal renal tubular acidosis:
      • PO 420-840mg/kg/day divided every 4-6 hours
  • Cardiac arrest:
    • Initially IV 1mEq/kg/dose *1
  • Urinary alkalinisation:
    • Initially PO 3900mg *1
    • Maintenance: PO 975-1950mg every 4 hours
    • Max: 15.6g/day
  • Dyspepsia:
    • < 60 years: PO 650-2600mg every 4 hours PRN
      • Max: 15.6g/day
    • >60 years: PO 650-1300mg every 4 hours PRN
  • Salicylate overdose:
    • IV 1-2 mEq/kg/dose *1
    • For patients with urinary pH <7.5
  • Tricyclic antidepressant overdose (Off-label):
    • IV 1-2 mEq/kg/dose *1
  • Tablet:
    • 300mg
    • 600mg
    • 1000mg
  • Injection:
    • 8.4%
  • 1 mEq = approx. 84mg
  • Stored at room temperature
  • Infuse over at least 2 hours for diluted solutions
  • Tablets can be taken whole with a glass of water or dissolved in water before swallowing
  • It increases serum bicarbonate, raising pH and also neutralizes gastric acidity
