- Acute depressive bipolar I disorder combined with fluoxetine:
- Adults: PO 5-12.5mg nocte
- Max 20mg/day
- Consider starting with PO 2.5mg nocte in non-smoker, debilitated, female patients or if there is a risk of hypotension
- Adults: PO 5-12.5mg nocte

- Acute treatment-resistant major depressive disorder:
- PO 5mg nocte
- Max: 20mg/day
- Used with fluoxetine
- Consider starting with PO 2.5mg nocte in non-smoker, debilitated, female patients or if there is a risk of hypotension
- Prevention of chemotherapy-associated nausea or vomiting (Off-label):
- Combined with dexamethasone and a 5-HT3 antagonist
- High emetic risk IV chemotherapy:
- PO 10mg the day of chemotherapy (day 1), followed by 10mg OD (days 2-4)
- PO 10mg the day of chemotherapy (day 1), followed by 10mg OD (days 2-4)
- Moderate emetic risk IV chemotherapy:
- PO 10mg the day of chemotherapy (day 1), followed by 10mg OD (days 2-3)
