• Schizophrenia:
    • Adults: Initially PO 5-10mg OD
      • May increase by 5mg/day
      • Initially PO 2.5-5mg OD in non-smoker, elderly, debilitated, female patients or if there is a risk of hypotension
      • Max 20mg/day
    • 13-17 years: PO 2.5-5mg/day initially
      • Target dosage: 10mg/day
      • Adjust by increments/decrements of 2.5-5 mg
      • Dosage range: 2.5-20mg/day
  • Manic or mixed episodes associated with bipolar I disorder:
    • Monotherapy: Initially PO 10-15mg OD
      • May adjust by 5mg/day PRN
      • Max: 20mg/day
  • Valproate & lithium adjunct:
    • Adults: Initially PO 10mg OD
      • May be adjusted by 5mg/day PRN
      • Max: 20mg/day
    • 13-17 years: PO 2.5-5mg/day initially
      • Target dosage: 10mg/day
      • Adjust by increments/decrements of 2.5-5 mg
      • Dosage range: 2.5-20mg/day
  • Agitation associated with schizophrenia and bipolar I mania:
    • IM 10mg *1
    • Max: 30mg/day
    • May repeat dose 2 hours after initial dose *1, then 4 hours after 2nd dose *1 PRN
    • May give lower doses of 2.5-7.5mg
    • Consider 5mg/dose in elderly and 2.5mg/dose in non-smoker, debilitated, female patients or if there is a risk of hypotension
    • Switch to PO as soon ass possible
  • Acute depressive bipolar I disorder combined with fluoxetine:
    • Adults: PO 5-12.5mg nocte
      • Max 20mg/day
      • Consider starting with PO 2.5mg nocte in non-smoker, debilitated, female patients or if there is a risk of hypotension
    • 10-17 years: PO 2.5mg nocte and fluoxetine PO 20mg nocte
  • Acute treatment-resistant major depressive disorder:
    • PO 5mg nocte
    • Max: 20mg/day
    • Used with fluoxetine
    • Consider starting with PO 2.5mg nocte in non-smoker, debilitated, female patients or if there is a risk of hypotension
  • Prevention of chemotherapy-associated nausea or vomiting (Off-label):
    • Combined with dexamethasone and a 5-HT3 antagonist
    • High emetic risk IV chemotherapy:
      • PO 10mg the day of chemotherapy (day 1), followed by 10mg OD (days 2-4)
  • Moderate emetic risk IV chemotherapy:
    • PO 10mg the day of chemotherapy (day 1), followed by 10mg OD (days 2-3)

  • Not approved for dementia-related psychosis.
