- Migraine headache with aura prophylaxis (Off-label):
- PO 100mg OD
- May initiate at 25mg OD *2/52 then increase by 25mg/day once a week
- Diabetic neuropathic pain (Off-label):
- PO 200-400mg divided OD/BD
- Initiate at 25mg OD *2/52, then 50mg/day divided OD/BD *2/52, then may increase dose once per week
