    • 3-12 years:
      • PO 10-15mg/kg/day divided TDS initially
      • Titrate up in approximately 3 days to effective maintenance dose

      • PO 25-35mg/kg/day divided TDS
    • >12 years (initial dose):
      • PO 300mg TDS
