- Tuberculosis (pulmonary and extrapulmonary)
- Continuation phase of TB treatment (after 2 months of RHZE or RHE)
- May be used in latent TB infection (LTBI) prophylaxis in some regimens
Dosage (based on weight):
- 30–37 kg:2 tablet
- 38–54 kg:3 tablets
- 55–70 kg:4 tablets
- >70 kg:5 tablets
- Administered once daily, usually for 4 months in continuation phase
- Take on an empty stomach
- Rifampicin: 150 mg
- Isoniazid: 75 mg
(Also available in 300 mg Rifampicin + 150 mg Isoniazid combinations)
- Take on an empty stomach, preferably 30–60 minutes before food
- Avoid antacids close to the dose
- Vitamin B6 (pyridoxine) supplementation is recommended to prevent neuropathy
- Rifampicin: Inhibits DNA-dependent RNA polymerase, bactericidal
- Isoniazid: Inhibits mycolic acid synthesis, essential for mycobacterial cell walls, bactericidal against actively growing bacilli
- Nausea, anorexia, fatigue
- Red/orange discoloration of body fluids (rifampicin)
- Peripheral neuropathy (isoniazid)
- Active liver disease
- Severe hepatic impairment
- Known hypersensitivity to rifampicin or isoniazid
- Rifampicin is a strong enzyme inducer:
- ↓ efficacy of oral contraceptives, warfarin, antiretrovirals
- Isoniazid may increase levels of phenytoin, carbamazepine
- Avoid alcohol → ↑ hepatotoxic risk
