- Overactive bladder (OAB) with symptoms of:
- Urinary frequency
- Urgency
- Urge incontinence
- Dosage:
- Immediate-release (IR): 1–2 mg twice daily
- Extended-release (ER):OD 4 mg
- IR tablets: 1 mg, 2 mg
- ER capsules: 2 mg, 4 mg
- Take with or without food
- Swallow ER capsules whole (do not crush/chew)
- Maintain consistent timing daily
- Stay hydrated, but avoid excess fluid before bedtime to reduce nighttime urination
- Class: Antimuscarinic agent
- Mechanism: Selectively blocks muscarinic receptors in the bladder → reduces detrusor muscle contractions, thereby controlling urination
- Onset: Gradual; may take weeks for full effect
- Metabolized in liver (CYP2D6 and CYP3A4)
- Dry mouth (most common)
- Headache, dizziness, constipation
- Blurred vision, urinary retention
- Rare: QT prolongation, tachycardia, confusion (especially in elderly)
- Urinary retention
- Uncontrolled narrow-angle glaucoma
- Gastric retention
- Severe hepatic impairment
- Caution with:
- CYP3A4 inhibitors (e.g., ketoconazole, clarithromycin)
- Other anticholinergics (↑ risk of side effects)
- May potentiate CNS effects when combined with sedatives
