• Overactive bladder (OAB) with symptoms of:
    • Urinary frequency 
    • Urgency
    • Urge incontinence
  • Dosage:
    • Immediate-release (IR): 1–2 mg twice daily
    • Extended-release (ER):OD 4 mg 
  • IR tablets: 1 mg, 2 mg
  • ER capsules: 2 mg, 4 mg
  • Take with or without food
  • Swallow ER capsules whole (do not crush/chew)
  • Maintain consistent timing daily
  • Stay hydrated, but avoid excess fluid before bedtime to reduce nighttime urination
  • Class: Antimuscarinic agent
  • Mechanism: Selectively blocks muscarinic receptors in the bladder → reduces detrusor muscle contractions, thereby controlling urination
  • Onset: Gradual; may take weeks for full effect
  • Metabolized in liver (CYP2D6 and CYP3A4)
  • Dry mouth (most common)
  • Headache, dizziness, constipation
  • Blurred vision, urinary retention
  • Rare: QT prolongation, tachycardia, confusion (especially in elderly)
  • Urinary retention
  • Uncontrolled narrow-angle glaucoma
  • Gastric retention
  • Severe hepatic impairment
  • Caution with:
    • CYP3A4 inhibitors (e.g., ketoconazole, clarithromycin)
    • Other anticholinergics (↑ risk of side effects)
  • May potentiate CNS effects when combined with sedatives
