• Toxoplasmosis:
    • Primary prophylaxis (Off-label): PO 50-75mg weekly
      • Not 1st-line agent
      • Given with dapsone, leucovorin
    • Treatment:
      • <60kg: PO 200mg *1, then 50mg OD for at least 6 weeks
        • Given with sulfadiazine and leucovorin or clindamycin and leucovorin
      • >60kg: PO 200mg *1, then 75mg OD for at least 6 weeks
        • Given with sulfadiazine and leucovorin or clindamycin and leucovorin
      • Children >2 months old: Loading dose: PO 2mg/kg/d divided BD *3/7
        • Maintenance: 1 mg/kg OD *4/52
    • Maintenance: (Off-label):
      • <60kg: PO 25mg OD
      • >60kg: PO 50mg OD
  • Congenital toxoplasmosis (>2 months):
    • Loading dose: 2mg/kg/day divided BD *2/7
    • Maintenance: 1mg/kg OD for 2-6 months old then remainder of 12 months old 1mg/kg 3 times/week
  • PCP prophylaxis (Off-label):
    • PO 50-75mg weekly
  • Cystoisosporiasis (Off-label):
    • Acute infection treatment
      • PO 50-75mg OD
      • Not 1st-line agent
      • Give with leucovorin
  • Chronic maintenance therapy:
    • PO 25mg OD
    • Not 1st-line agent
    • Give with leucovorin

Tablet: 25mg

  • To be taken with meals
  • Take with a lot of water if taking with a drug with a sulfa component
