Indications:

  • Allergic rhinitis (seasonal and perennial)

  • Chronic urticaria

  • Asthma (adjunct in mild persistent cases) — Montelukast component helps with bronchospasm

  • Effective for nasal congestion, sneezing, runny nose, itching, and wheezing/cough from allergic triggers

Dosage (Adult and Adolescent ≥12 years):

  • Fexofenadine 120 mg + Montelukast 10 mg — once daily (usually in the evening)

Pediatric Dosage:

  • Varies by age and formulation; usually lower doses (e.g., Fexofenadine 30 mg + Montelukast 4/5 mg for children)

Common fixed-dose combination:

  • Fexofenadine 120 mg + Montelukast 10 mg (once daily)

  • Pediatric versions: e.g., Fexofenadine 30 mg + Montelukast 4 mg

  • Take once daily, preferably in the evening

  • Can be taken with or without food

  • Avoid fruit juices and antacids near the time of taking fexofenadine

Fexofenadine:

  • Class: Second-generation antihistamine (H1 receptor antagonist)

  • Mechanism: Blocks peripheral H1 receptors → reduces allergic symptoms

  • Does not cross blood-brain barrier significantly → minimal sedation

Montelukast:

  • Class: Leukotriene receptor antagonist

  • Mechanism: Blocks leukotriene D4 at the cysteinyl leukotriene receptor → reduces inflammation, bronchoconstriction, and mucus production

Fexofenadine:

  • Headache

  • Drowsiness (rare)

  • Nausea

  • Dizziness

Montelukast:

  • Headache

  • Abdominal pain

  • Fatigue

  • Neuropsychiatric events (rare but serious): anxiety, agitation, depression, suicidal thoughts

  • Known hypersensitivity to either fexofenadine or montelukast

  • History of neuropsychiatric events (caution with montelukast)

  • Severe renal impairment (adjust fexofenadine if necessary)

  • Montelukast:

    • Metabolized by CYP3A4, CYP2C9 → caution with rifampin, phenytoin, phenobarbital (may reduce effectiveness)

  • Fexofenadine:

    • Absorption reduced by antacids containing aluminum or magnesium (avoid within 2 hours)

    • Avoid grapefruit/apple/orange juice (can reduce bioavailability)
