• Type II DM:
    • Conversion from sitagliptin plus metformin: 1 tab BD
      • If on metformin 850mg BD, start 50mg/1000mg BD
      • Max: 100mg/2000mg/day
    • Conversion from sitagliptin: Initially 50mg/500mg BD
      • Max: 100mg/2000mg/day
    • Conversion from metformin: Sitagliptin 50mg BD and current daily dose of metformin
      • If on metformin 850mg BD, start 50mg/1000mg BD
      • Max: 100 mg/2000 mg/day
  • Tablet:
    • 50/500mg
    • 50/850mg
    • 50/1000mg

To be given with meals

Sitagliptin: (DPP-4) inhibitor; It inhibits dipeptidyl peptidase-4, slowing incretin metabolism, increasing insulin synthesis/release thereby decreasing glucagon levels

Metformin: Biguanide; It delays the absorption of glucose from the GI tract and increase insulin sensitivity in the muscle thereby improving peripheral glucose uptake and utilization in cells.

WARNING:

Lactic acidosis: post-marketing cases of hypothermia, hypotension, resistant bradyarrhythmias and death reported. If suspected, discontinue and hospitalize the patient immediately
