- Type 2 DM:
- Initial dose: PO 2.5mg/500mg OD/ BD
- Max: 20mg/2000mg daily, divided into multiple doses.
Tablet:
- 2.5mg/500mg
- 5mg/500mg
- Take with meals to reduce gastrointestinal side effects
Glibenclamide: Sulfonylurea. It stimulates insulin secretion from functioning pancreatic beta cells, which lowers blood glucose levels.
Metformin: Biguanide. It decreases hepatic glucose production, reduces intestinal glucose absorption, and improves insulin sensitivity by increasing peripheral glucose uptake and utilisation.
- Hypoglycemia
- Weight gain
- Gastrointestinal disturbances (nausea, vomiting, diarrhoea)
- Metallic taste
Black Box warning:
Risk of lactic acidosis due to metformin accumulation, especially in patients with renal impairment, liver disease, or conditions that increase lactate production.
- Severe renal impairment
- Hepatic impairment
- Hypersensitivity to components
- Type 1 DM
- Diabetic ketoacidosis.
- Alcohol
- Diuretics
- Corticosteroids
- Beta-blockers
- ACE inhibitors
- Radiologic contrast agents
