• Methotrexate overdose:
    • IM/IV 10mg/m2/dose every 6 hours
  • High dose methotrexate rescue:
    • IV 10mg/m² every 6 hours *10 doses
    • Starts 24 hours after beginning of methotrexate infusion
  • Folate antagonist overdose (pyrimethamine/trimethoprim):
    • PO 10mg/m2 every 6 hours
  • Megaloblastic anemia due to folate deficiency:
    • IM/IV <1mg OD
    • Max: 1mg/day

Injection:

  • 10mg/mL
  • 7.5mg/mL
  • Infusion not to exceed 160mg/min
  • Reconstituted solution is stable for 7 days
  • It counteracts folic acid antagonists such as methotrexate since it is an active metabolite of folic acid
  • It displaces methotrexate from intracellular binding sites and restores the folate required for DNA/RNA synthesis
  • In methanol toxicity, it serves as a tetrahydrofolate source to help the body eliminate the formic acid resulting from methanol’s toxicity
