- Intra-abdominal infection (appendicitis/peritonitis):
- Adults: IV 3.375g every 6 hours *4-7 days
- 2-9 month old: IV 270mg/kg/day divided TDS *4-7 days
- 9 months and older (<40 kg): IV 337.5mg/kg/day divided TDS *4-7 days
- 9 months and older (<40 kg): IV 3.375g every 6 hours *4-7 days
- Gynecologic infections (PID or postpartum endometriosis):
- IV 3.375g every 6 hours *7-10 days

- Skin & soft tissue infections:

- Community-acquired pneumonia:

Renal Modifications:
- If usual dose 3.375g every 6 hours:
- CrCl 20-40 mL/min: 25g every 6 hours
- CrCl <20 mL/min: 2g TDS
- Hemodialysis: 25g BD
- Give 0.75g as supplement after dialysis if next maintenance dose not due right after dialysis
- Peritoneal dialysis: 25gBD
- No supplement
- If usual dose 4.5g every 6 hours:
- CrCl 20-40 mL/min: 375g every 6 hours
- CrCl <20 mL/min: 25g every 6 hours
- Hemodialysis: 25g TDS
- Give 0.75g as supplement after dialysis if next maintenance dose not due right after dialysis
