- Susceptible bacterial infections:
- Conventional interval dosing: IV/IM 3-5 mg/kg/day IV/IM divided TDS
- Extended interval dosing: IV 5-7 mg/kg/dose OD
- Not indicated in pregnancy, burns, ascites, obesity, neutropenia or endocarditis
- Infants: IV/IM 2.5mg/kg/dose TDS
- Children and adolescents: IV/IM 2-2.5mg/kg/dose TDS
- Gram-positive bacterial endocarditis:
- IV 3mg/kg/dose OD for at least *2/52
- Used in combination with other antibacterials
- Uncomplicated gonococcal infections (infections of cervix, urethra, rectum):
- IM 240mg *1
- Given with azithromycin
- Not 1st line
- Severe PID:
- IM/IV 3-5mg/kg/dose OD *14/7 OR IM/IV 2mg/kg/dose*1, then 1.5mg/kg/dose TDS *14/7
- Given with clindamycin
- Not 1st line agent
- Hospital-acquired pneumonia:
- IV 5-7mg/kg/dose OD *1/52
- May extend dose based on clinical response
- UTI (pyelonephritis):
- IV 5-7mg/kg/dose IV *1
- To complete treatment, give ciprofloxacin, levofloxacin or sulfamethoxazole/trimethoprim
- Surgical prophylaxis:
- IV 5mg/kg *1 60min prior to surgical incision
- IV 1.5 mg/kg *1 for gynecology procedures
- Brucellosis (Off-label):
- IM/IV 5mg/kg/dose OD *7-10 days
- Injection/infusion:
- 10mg/mL
- 40mg/mL
- 80mg/mL
- Infuse over 30 min-2 hours
- After infusion, flush line with NS or D5W
Aminoglycoside
It binds to bacterial 30S ribosomal subunit thereby inhibiting protein synthesis

WARNING:
- Risk of neurotoxicity/ototoxicity with renal impairment, high-dose and prolonged treatment with ototoxicity usually irreversible. Avoid concurrent and/or sequential neurotoxic agents and potent diuretics
- Risk of nephrotoxicity with renal impairment, high-dose and prolonged treatment. Avoid concurrent and/or sequential nephrotoxic agents and potent diuretics
- Risk of neuromuscular blockade including respiratory paralysis
