• Staphylococcal infections resistant to benzylpenicillin:
    • Adult:
      • IM/IV 1,000-2,000mg QID
      • May be increased to 2,000 mg 4 hourly in more severe infection OR
      • PO 250-500mg QID
      • Max: 6,000mg daily
    • Neonates ≤7 days old:
      • <2 kg: IV 25mg/kg BD
      • ≥2 kg: IV 25mg/kg TDS
    • 7-28 days:
      • <2 kg: IV 25mg/kg TDS
      • ≥2 kg: IV 25mg/kg QID
      • For treatment of meningitis, doses may be doubled
    • ≥1 month:
      • <20 kg: IV/IM 50-100 mg/kg daily (Max: 4,000 mg) divided QID
      • May be increased for severe infections up to 200mg/kg daily (Max: 12,000mg) in divided doses
    • Child ≤20 kg: PO 25-50 mg/kg daily in divided doses QID

  • Should be taken on an empty stomach; 1 hour before or 2 hours after meals

Isoxazolyl penicillin

A bactericidal antibiotic that binds to 1 or more of the penicillin-binding proteins (PBPs) which in turn inhibit the final transpeptidation step of peptidoglycan synthesis in bacterial cell walls, thereby inhibiting cell wall synthesis. Bacterial lysis eventually occurs due to the ongoing activity of cell wall autolytic enzymes (autolysins and murein hydrolases) while cell wall assembly is arrested.
