• Group A Streptococcal upper respiratory infections e.g. pharyngitis, tonsillitis:
    • Adults: 2 million units IM *1
    • Older children: 900,000 units *1
    • Infants and younger children: 300,000-600,000 units *1 
  • Syphilis; primary, secondary or latent <1 year:
    • 4 million units IM *1; may repeat dose after 1 week for pregnant patients
  • Syphilis; tertiary, latent >1 year or unknown duration:
    • 4 million units IM per week *3/52 
  • Rheumatic fever prophylaxis:
    • 2 million units every 4 weeks or 600,000 units every 2 weeks

Injection:

  • 2.4 MU
  • NOT to be used IV.
  • Should NOT be mixed with other IV solutions.
  • Do NOT inject near nerve or artery.
  • Deep IM use
  • Administered only in the upper or outer quadrant of dorsogluteal or ventrogluteal site

Penicillin antibiotic.

It inhibits cell wall mucopeptide synthesis especially during active multiplication hence bactericidal.

  • Serum-sickness like reaction (fever, chills, edema, arthralgia, prostration)
  • Jarisch-Herxheimer reaction

  • Probenecid- prolongs half-life of Pen G. May be used for this purpose.
