Ondansetron

• Prevention of postoperative nausea and vomiting:
  • Adults:
    • PO 8mg 1 hour prior to anesthesia then 8mg at 8 hour intervals OR slow IV 4mg at anesthesia induction. 

• • Children (1 month- 12 years):
    • <40kg: slow IV single dose 0.1mg/kg 

• • • >40kg: slow IV single dose 4mg 

• Prevention of chemotherapy-induced nausea and vomiting:
  • Adults:
    • IV single dose 32mg infused over 15 minutes, 30 minutes before start of emetogenic chemotherapy.

OR

• • • IV three-dose regimen of 0.15 mg/kg over 15 min administered 30 min before chemotherapy, then 4 and 8 hours after first dose. Max 16mg

OR

• • • PO highly emetogenic: 24 mg 30 minutes before chemotherapy. 

• • • PO moderately emetogenic: : 8 mg started 30 minutes before chemotherapy, then 12 hourly for 1-2 days after chemotherapy. 

• • Children:
    • > 6months; highly or moderately emetogenic: 15 mg/kg/dose IV 4 hourly * 3 doses, 30 min before chemo. 

• • • 4-11 years, moderately emetogenic: PO 4mg 4 hourly for 3 doses, 30 min before chemo. Then 4 mg PO 8 hourly until 1-2 days after chemo is completed. 

• • • >12 years, moderately emetogenic: PO 8 mg 8 hourly for 2 doses, then 8 mg PO 12 hourly until 1-2 days after chemo is completed. 

• Hyperemesis gravidarum:
  • IV 10 mg TDS PRN
