• General anesthesia induction:
    • Healthy adults <55 years:
      • IV 2-2.5mg/kg/dose given as 40mg every 10 sec until induction onset
    • >55 years/debilitated:
      • IV 1-1.5mg/kg/dose given as 20mg every 10 sec until induction onset
    • Neurosurgical patients:
      • IV 1-2mg/kg/dose given as 20mg every 10 sec until induction onset
    • Cardiac anesthesia patients:
      • IV 0.5-1.5mg/kg/dose given as 20mg every 10sec until induction onset
    • Children (3-16 years):
      • IV 2.5-3.5mg/kg/dose *1
  • Maintenance of general anesthesia:
    • Healthy adults <55 years:
      • IV 100-200mcg/kg/min OR 25-50mg PRN
    • >55 years/debilitated:
      • IV 50-100mcg/kg/min
    • Neurosurgical patients:
      • IV 100-200mcg/kg/min
  • Cardiac anesthesia patients:
    • IV 100-150mcg/kg/min
    • Decrease dose to 50-100mcg/kg/min if used as secondary agent
    • Children (2 months-16 years):
      • IV 125-300mcg/kg/min
  • Monitored anesthesia care:
    • Induction:
      • IV 100-150mcg/kg/min *3-5min OR IV 0.5mg/kg/dose *1
    • Maintenance:
      • IV 25-75mcg/kg/min OR IV 10-20mg PRN
      • Avoid bolus doses and reduce dose 20% in elderly, debilitated or neurosurgical patients
  • ICU sedation:
    • IV 5mcg/kg/min *5min, then may increase by 5-10mcg/kg/min every 5-10min
      • Allow a minimum of 5 min between adjustments for onset of peak effect
    • Maintenance: 5-50mcg/kg/min

Injection: 1% w/v

  • To reduce pain associated with injection, use larger veins of forearm or antecubital fossa; lidocaine IV (1mL of a 1% solution) may also be used prior to administration

Short-acting, lipophilic sedative/hypnotic general anesthetic

It is thought to causes global CNS depression through agonist actions on GABA receptors
