• Malignant ventricular arrhythmias:
    • Loading dose: PO 800-1600mg OD *1-3 weeks until response
    • Once adequate arrhythmia control achieved, reduce dose to 600-800mg/day *1/12
    • Then reduce to maintenance dose PO 400mg OD
    • IV 150mg *1 over 10min, then 1mg/min *6h, then 0.5mg/min *18h
  • ACLS, Pulseless Ventricular Fibrillation/Ventricular Tachycardia (Off-label):
    • IV/ intraosseous push 300mg after dose epinephrine if no initial response to defibrillation
    • May follow initial dose with 150mg every 3-5min

Class III antiarrhythmic agent

It inhibits adrenergic stimulation. It affects sodium, potassium and calcium channels and markedly prolongs action potential and repolarization. It also decreases AV conduction and sinus node function
