    • Intensive phase:
      • <59 years: IM 15mg/kg/dose 5-7x/week. Max: 1g/dose
      • ≥59 years: IM 10mg/kg/dose 5-7x/week. Max: 750mg/dose
      • Children: IM 20-40mg/kg/dose 5-7x/week. Max: 1 g/dose
    • Continuous phase:
      • <59 years: IM 15mg/kg/dose 2-3x/week. Max: 1g/dose
      • ≥59 years: IM 15mg/kg/dose 2-3x/week. Max: 750mg/dose
      • Children: IM 20-40mg/kg/dose 2-3x/week. Max: 1g/dose
  • Enterococcal endocarditis:
    • IM 1g BD *2/52, then 500mg BD *4/52, concomitant with penicillin
    • Max: 1g/dose
  • Plague:
    • Adults: IM 15mg/kg BD *10/7. Max: 1g/dose
    • Children: IM 15mg/kg/dose BD *10/7
  • Tularemia:
    • IM 7.5-15mg/kg/dose BD *10/7
    • Max: 2g/day
  • Streptococcal endocarditis:
    • IM 1g BD *1/52, then 500mg BD *1/52, concomitant with penicillin
    • If >60 years old, 500mg BD *14/7
  • Brucellosis:
    • Adults: IM 1g OD/BD *1/52, then OD *1/52 week in conjunction with doxycycline or tetracycline
    • >8 years old: IM 20mg/kg divided BD during 7-14 days of tetracycline or co-trimoxazole therapy. Max: 1g/dose

  • Not recommended to be given IV
  • Reconstituted solution stable for 1 week at room temp

WARNING

  • It may cause nephrotoxicity and neurotoxicity. Avoid concurrent use of nephrotoxic/neurotoxic drugs.
  • Neuromuscular blockade including respiratory paralysis may occur, especially when given soon after anesthesia or muscle relaxants
