- To decrease infection incidence in patients receiving myelosuppressive chemotherapy treatment associated with a significant incidence of severe febrile neutropenia:
- SC/IV infusion 5mcg/kg/day until neutrophil count in normal range (about 14-38 days)
- Started at least 24 hours after cytotoxic chemotherapy
- Preferably given SC
- If given IV, administer over 30 minutes
- SC/IV infusion 5mcg/kg/day until neutrophil count in normal range (about 14-38 days)
- Reduction in duration of neutropenia (and associated sequelae) in myeloablative therapy followed by bone marrow transplantation:
- SC/IV infusion 10mcg/kg/day over 4-24 hours (Max 24 hour infusion)
- Peripheral blood progenitor cell collection by leukapheresis:
- Monotherapy: SC/IV infusion 10mcg/kg daily *5-7 days
- Following adjunctive chemotherapy: SC/IV infusion 5mcg/kg daily
- To be administered over 24 hours if given SC
- Started 4 days before 1st leukapheresis and continued through last leukapheresis
- Severe chronic neutropenia:
- Congenital: Initial SC 6mcg/kg BD
- Idiopathic/cyclic: Initial SC 5 mcg/kg OD
- Hematopoietic syndrome of acute radiation syndrome:
- SC 10mcg/kg OD
- SC 10mcg/kg OD
- Persistent neutropenia in HIV infection:
- Initially SC 1mcg/kg OD
- Dose increased according to neutrophil count
- Max: 4mcg/day
- Initially SC 1mcg/kg OD
- Infusion/injection:
- 6mg/0.6mL
- 300mcg
- Pre-filled syringe:
- 30mcg
- To be refrigerated
- Discard any vial or prefilled syringe left at room temperature for >24 hours
- Avoid shaking
- Rotate injection site daily
- Do not inject into an area that is tender, red, bruised or hard or that has scars or stretch marks
