• Strongyloidiasis of the intestinal tract:
    • 15-24 kg: 3mg once
    • 25-35 kg: 6mg once
    • 36-50 kg: 9mg once
    • 51-65 kg: 12mg once
    • 66-79 kg: 15mg once
    • >80 kg: 200mcg/kg once
  • Onchocerciasis (River blindness)
    • 15-25 kg: 3mg
    • 26-44 kg: 6mg
    • 45-64 kg: 9mg
    • 65-84 kg: 12mg
    • ≥85 kg: 150 mcg/kg
    • May repeat in 3-12 months
    • Doesn’t treat adult worms; must be surgically removed
  • Scabies (Off-label):
    • Non-crusted:
      • Adults & >15kg: 2mg/kg/dose every 2 weeks *2 doses
    • Crusted:
      • Adults & >15kg: 2mg/kg/dose *1 on days 1, 2, 8, 9, 15
      • May also give on days 22 and 29 for severe cases
      • Give with permethrin topical 5% cream
      • Give with food
  • Pediculosis capitis (head lice) (Off-label):
    • 200 mcg/kg once
    • May require 1-2 additional doses repeated after 7 days
    • Give with food
  • Pediculosis corporis (Body lice)(Off-label):
    • 12mg every 7 days *3 doses
    • Give with food
  • Pediculosis pubis (Pubic lice)(Off-label):
    • 25mg/kg/dose every 1-2 weeks *2 doses
    • Give with food

Tablet:

  • 6mg
  • 3mg

Taken on an empty stomach at least 1 hour before meals unless indicated

It increases invertebrate nerve and muscle cell membrane permeability by binding to chloride ion channels which results in paralysis and death of parasite
