• Systemic fungal infection such as Aspergillus spp, Candida spp and/or Cryptococcus spp:
    • IV 3-5 mg/kg/dose every 24 hours
  • Febrile neutropenia in empiric therapy for presumed fungal infection:
    • IV 3-5 mg/kg/dose every 24 hours
  • Cryptococcal meningitis:
    • IV 6mg/kg/dose every 24 hours *11-21 days
  • Visceral leishmaniasis:
    • Immunocompetent patients: IV 3mg/kg/dose every 24 hours on days 1-5, 14, 21
    • Immunocompromised patients: IV 4mg/kg/dose every 24 hours on days 1-5, 10, 17, 24, 31, 38
