• Field block:
    • Infiltration anaesthesia: Up to 15ml (up to 150mg lignocaine) of lignocaine 1% strength with adrenaline.
    • Intercostals per nerve: 2-5mL (20-50mg lignocaine, max 150mg) of lignocaine 1% strength with adrenaline.
      • Max 15mL
  •  Field block for instrumental delivery:
    • Pudendal: 10mL (100mg lignocaine) of lignocaine 1% strength with adrenaline.
  •  Major nerve block for surgical operations:
    • Sciatic: 15mL (300mg lignocaine) of lignocaine 2% strength with adrenaline.
  •  Major nerve block for surgical operations, dilation of the cervix and obstetric pain relief:
    • Paracervical (each side): 10mL (100mg lignocaine) of lignocaine 1% strength with adrenaline.
  • Injection:
    • 2% + 1:200,000
    • 2% + 1:80,000
  • Doses should be injected slowly at a rate of 100-200mg/minute or in increments.
  • Duration of block (with adrenaline) is about 90 minutes.
  • Maximum single dose of lignocaine is 500mg when given with adrenaline.

Lignocaine (lidocaine): Amide local anesthetic. It inhibits Na ion channels, stabilizing neuronal cell membranes and inhibiting nerve impulse initiation and conduction. It also works by depressing the action potential phase 0 as a class Ib antiarrhythmic

Adrenaline (epinephrine): Sympathomimetic. It stimulates alpha and beta adrenergic receptors
